Class Drop Request Form

We are sorry that you have decided to stop taking classes with us here at Young’s. However, we understand that there are times when dropping a class is necessary. Please fill out all of the information below and submit the form to complete your drop request. We would greatly appreciate it if you would let us know if there was something you didn’t like about our program when giving your drop reasons below. Over the years we have learned that even though we never like to get complaints, we can’t fix a problem if someone doesn’t let us know! Please be honest and we will keep your input confidential. If dropping more than one child, still just use this one form to save time!

    I understand that once this document is submitted to the Young's Gymnastics & Cheerleading, Inc. business office that my drop request goes into effect immediately for the following month. If I submit this form prior to the end of the month I understand that I will not receive any credits or refunds for the remaining classes in the current month. I do understand that my children are able to complete the remaining classes that I have paid for in the current month. I also understand that any missed classes MUST be made up during a paying month. Once I submit this form I understand that any missed classes that have not been made up before this drop date are forfeited!
